security: remove Claude session logs from repo 
- Delete 'Claude Code' conversation transcript (~1.6 MB)
- Add Claude Code/Claude Agent patterns to .gitignore

These files contained internal workspace paths, code diffs, and session context
that should not be versioned or public.
